How to Request

  1. You can apply through WhatsApp, LINE, or our contact form.
  2. If you choose postal delivery, actual shipping costs will apply.
  3. For international shipping, documents will be sent via EMS; standard postal services are not available.
  4. Payment must be made in advance.
  5. For in-person payments at the hospital, you can use a credit card or pay in cash.
  6. For requests from outside the hospital, including overseas, payment can only be made by bank transfer; credit cards are not accepted. Document processing will start once your payment has been confirmed.
  7. Please contact us for information on document issuance fees.
  8. It typically takes 1 to 2 weeks for documents to be ready for pick-up.
  9. For international shipping, please allow an additional 10 to 14 days.

Fit to Fly Health Certificate

Outpatients

  • If your departure is more than 5 hours after your visit, we can issue the certificate on the same day.
  • If your airline requires a specific form, please provide it before your medical consultation.
  • The certificate is usually ready a few hours after your consultation.

Inpatients

  • As soon as your flight details are confirmed, we’ll start preparing your Fit to Fly Health Certificate.
  • Please notify us immediately once your flight is confirmed.

Medical Imaging Fees (Provided on CD-ROM)

  • The CD includes viewer software for accessing the image data (DICOM files).
  • The hospital is not responsible for any issues related to software use.
  • General X-ray:5,500 JPY
  • CT or MRI Image:8,800 JPY
  • Gastroscope/Endoscope,CT, & MRI Images:11,000 JPY
