At our hospital, we offer a full range of treatment options, including surgery, drug therapy, and radiation therapy.
Surgery
We provide breast-conserving surgery, mastectomy, sentinel lymph node biopsy, lymph node dissection, and breast reconstruction using autologous tissue.
During breast-conserving surgery and sentinel lymph node biopsy, we collaborate with pathologists to examine the surgical margins and determine the presence of lymph node metastasis during the procedure.
For sentinel lymph node biopsy, we aim to minimize unnecessary lymph node removal, which can lead to uncomfortable complications, by following the results of the Z-0011 trial (which suggests no additional lymph node removal is needed if there are up to two macroscopic metastases) and the AMAROS trial (which indicates that if clinical lymph node metastasis is negative, radiation therapy after sentinel lymph node metastasis is sufficient without additional lymph node removal). Our experience with sentinel lymph node biopsy dates back to July 2000, before it was covered by insurance, and we have conducted over 2,500 cases and numerous studies on the procedure.
Breast Reconstruction Using Autologous Tissue
We offer not only the commonly performed latissimus dorsi flap reconstruction, which is often used alongside breast-conserving surgery requiring relatively large tissue excision, but also extended latissimus dorsi flap procedures (using fat and muscle from the waist area) for cases where a mastectomy is performed (often preserving the nipple) and where a larger breast volume is desired without the need for postoperative radiation therapy. This method resolves the typical issue of insufficient volume in reconstructed breasts with the standard latissimus dorsi flap while maintaining the stable blood flow advantages of this technique. Despite a minor drawback of visible scarring on the back that may not be covered by a bra, this is a highly effective procedure. The technique is well-established, and patients at Kishiwada Tokushukai Hospital can confidently undergo this procedure.
Nipple-Sparing Surgery
Based on extensive case analysis, we have not only presented our findings at conferences but also authored numerous books and papers. In situations where nipple removal would be the standard at most facilities, our innovative surgical techniques often allow for nipple preservation. Nipple-sparing surgery poses challenges, such as the risk of nipple necrosis (tissue death) and recurrence in the preserved nipple. However, our research has shown no cases of nipple necrosis, and the treatment outcomes are comparable to those of mastectomies with nipple removal, making nipple-sparing surgery a safe option.
Drug Therapy
Our drug therapy, especially treatments involving chemotherapy and molecularly targeted therapy, is carried out according to standard protocols. However, in cases where curative treatment is generally considered difficult, we may explore curative options in close consultation with the patient, and we have experienced many cases where we achieved what we believe to be a cure. Since opinions on this area vary significantly even among breast cancer specialists, please feel free to consult with us. Although I am a surgeon, I have extensive experience in drug therapy and can offer a range of treatment options. Additionally, since our hospital provides radiation therapy, we aim to offer the best possible treatment for recurrent or metastatic breast cancer through a multidisciplinary approach. We have also been committed to the diagnosis and treatment of bone metastases for many years and have conducted various studies in this area.
