We issue medical documents and certificates related to your visit or admission.
English documents are available upon request (fees apply).

Fit to Fly Health Certificate (English)

Some travellers are asked by the airport or airline to provide a medical certificate before boarding.
We can assess your condition and issue a Fit to Fly Health Certificate when medically appropriate.

  • Outpatients:
    If your departure is more than 5 hours after your visit, the certificate may be issued on the same day.
  • Inpatients:
    Once your flight details are confirmed, we will begin preparing the certificate.

Fit to Fly certificates are issued only after an in-person consultation and medical assessment.
Same-day issuance is not guaranteed.

How to Request

If you request postal delivery, actual shipping costs will apply.
For international shipping, documents will be sent via EMS; standard postal services are not available.

  • Full name (as shown on your passport)
  • Date of birth
  • Passport number
  • Airline name
  • Flight number
  • Departure date & time
  • Reason for request
  • Preferred visit date/time

If Your Airline Requires a Specific Form

You must provide the official PDF file or direct download URL before your visit.

  • Please send the file digitally in advance.
  • Printed copies are not accepted.
  • We are unable to obtain airline forms on your behalf.

Payment & Delivery

Advance payment is required.

  • In-person: Credit card or cash (JPY)
  • Outside hospital: Bank transfer only
  • Shipping: EMS for international delivery

Processing Time

  • Standard documents: 1–2 weeks
  • International shipping: additional 10–14 days
  • Fit to Fly: same-day possible (conditions apply)
